TSI Healthcare provides each team member with a company identification (ID) badge. Complete the form below in its entirety to request your ID badge.

Team Member Identification (ID) Badge Procedure
As a vital part of our office security, a TSI Healthcare identification badge with your first name, photo and department is being issued to you. The ID badge also serves as your electronic key to enter the building and other secured areas as needed. ID badges provide a visible means of identification for team members and help us know who belongs in the office and who does not.
- All team members are required to wear a TSI Healthcare badge in plain view while in the office. Failure to wear your ID badge, or excessive loss or damage to cards may lead to disciplinary action.
- The ID badge may be used only by the individual to whom it was issued. Team members may not “loan” their ID badge to anyone for any reason.
- Team members shall surrender the ID badge to their supervisor upon termination of employment, beginning an extended leave of absence, or when requested.
- If your identification badge is lost or stolen, notify the Director of Operations and your direct supervisor immediately (call 800.354.4205 or email smoles@tsihealthcare.com).
- Replacement ID badges will be charged back to you at a rate of $20/badge.
- If a team member transfers from one department to another a new ID badge will be issued.
- Team members observing people in the office without ID should immediately report the person to any supervisor.
- The ID badge may be worn attached to a clip or on a lanyard, and must be clearly visible.
